Kindergarten and PreK Screening 2022-23 @ BPS

Time: 9:00 am - 3:00 pm

Kindergarten Registration for the 2022-2023 School Year is March 24, 2022.
REQUIRED REGISTRATION DOCUMENTS
A copy of the original birth certificate.
A completed health assessment and dental screening. Must be completed by a doctor.
A copy of the Immunization Record
A copy of the Social Security card.
Child must be 5 years old on or before August 31, 2022.

PREK screening is scheduled for April 11 and April 12 at Bladenboro Primary School:
REQUIRED REGISTRATION DOCUMENTS
1. Child must be 4 years old on or before August 31, 2022
2. Proof of residence (electric bill, water bill, lease, etc.)
3. A copy of the original Certified Birth Certificate
4. A completed health assessment & dental screening (must be completed by a
doctor)
5. A copy of the Immunization Record
6. A copy of the Social Security Card
7. Proof of income verification documents
a. Tax records
i. W-2’s
ii. 1040-Adjusted Gross line 7
b. Pay stubs (most recent pay periods)
i. Weekly – submit four consecutive pay stubs
ii. Bi-weekly - submit two consecutive pay stubs
iii. Monthly - submit at least one full month’s pay stub
